Wellspring Care Portal complies with applicable federal civil rights laws and does not discriminate on the basis of race, color, national origin, age, disability, sex, sexual orientation, gender identity, gender expression, religion, creed, marital status, genetic information, veteran status, citizenship status, or any other characteristic protected by applicable law.
This commitment applies to all programs, services, and activities provided by Wellspring Care Portal, including but not limited to:

Applicable Laws
Wellspring Care Portal operates in accordance with the following federal civil rights laws and regulations:
- Section 1557 of the Affordable Care Act (ACA): Prohibits discrimination on the basis of race, color, national origin, sex (including pregnancy, sexual orientation, gender identity, and sex characteristics), age, or disability in covered health programs and activities.
- Title VI of the Civil Rights Act of 1964: Prohibits discrimination on the basis of race, color, or national origin in programs and activities receiving federal financial assistance.
- Section 504 of the Rehabilitation Act of 1973: Prohibits discrimination on the basis of disability in programs and activities receiving federal financial assistance.
- Title III of the Americans with Disabilities Act (ADA): Prohibits discrimination on the basis of disability in places of public accommodation, including healthcare facilities.
- Age Discrimination Act of 1975: Prohibits discrimination on the basis of age in programs and activities receiving federal financial assistance.
Reasonable Accommodations and Modifications
Wellspring Care Portal provides reasonable accommodations and modifications to ensure that individuals with disabilities have equal access to our services. Accommodations may include, but are not limited to:

How to File a Discrimination Complaint

Complaints may be submitted in writing, by phone, or in person. If you need assistance filing a complaint, our staff will help you. You may also file a complaint on behalf of another person.

Complaints must generally be filed within 180 days of the date of the alleged discrimination. OCR may extend the filing period for good cause.
Retaliation Prohibited
Wellspring Care Portal does not retaliate against any individual for filing a complaint, participating in a complaint investigation, or opposing any practice prohibited by applicable civil rights laws. Any person who believes they have been subjected to retaliation should immediately contact our Civil Rights Coordinator or file a complaint with the Office for Civil Rights.
